How to Prepare Your DSS License Application for an RCFE or ARF

Week 3 of our RCFE/ARF Startup Series: From LLC to First Client

Submitting a complete, accurate DSS license application is one of the most important milestones in your RCFE or ARF journey. Incomplete applications are returned and restart the timeline. Here’s everything you need to include.

The Core Application Form: LIC 200

The LIC 200 (Application for Community Care Facility License) is the primary DSS application form. It captures your facility name, address, ownership structure, administrator information, and requested capacity. Download it from the California DSS website and complete every field carefully — gaps trigger automatic return.

Complete Application Checklist

Business & Ownership Documents

  • LLC Operating Agreement or Articles of Organization
  • List of all owners/members with percentage of ownership
  • Financial statement demonstrating fiscal solvency (typically 2–3 months of operating expenses in reserve)

Facility Documents

  • Lease or deed for the facility
  • Floor plan (drawn to scale, showing room dimensions and use)
  • Local fire clearance approval letter
  • Proof of zoning compliance (or conditional use permit if required)

Administrator Qualifications

  • RCFE: Proof of 80-hour DSS-approved Administrator Certification + exam passage
  • ARF: Documentation of education and experience meeting DSS Title 22 requirements for your facility size
  • Current TB clearance for administrator

Background Clearances

  • LiveScan fingerprint clearance for all owners, operators, and household members (if residing at facility)
  • Criminal record statement (LIC 508) for each person requiring clearance

Policies & Procedures Manual

This is the most time-consuming part of the application — and the most commonly deficient. Your P&P manual must cover:

  • Admission, retention, and discharge criteria
  • Resident rights
  • Medication management (storage, administration, documentation)
  • Staffing plan and ratios
  • Emergency and disaster plan
  • Reporting of incidents and injuries
  • Behavior support (especially for ARFs serving mental health or DD populations)
  • Grievance procedures

Additional Required Items

  • TB clearances for all staff and household members
  • Proof of liability insurance
  • First aid and CPR certification for administrator and key staff

After You Submit

The DSS CCLD will review your application for completeness. If anything is missing, they’ll send a deficiency notice and your timeline resets. If complete, they’ll schedule your pre-licensing inspection (covered in Week 4).

Most regional CCLD offices take 4–12 weeks to schedule an inspection after receiving a complete application. Use that time to prepare.

Pro Tip: Submit in Person When Possible

Some regional CCLD offices allow or prefer in-person submission. This lets you confirm completeness on the spot and build a relationship with your licensing analyst — which can smooth the process going forward.
